Surveys can be added as an embedded button to any page you have made, simply navigate to the page you wish to add the survey access from.
Select Edit Page -> Insert Item -> Surveys
Choose where you would like the survey widget to be on your page and site will present you with options of your various created surveys, simply choose the appropriate survey and a widget will be created directing people to the standalone survey page.
